This notice describes the policy of Actio Fitness LLC, Doing Business As Evan Wes wellness (hereafter Evan Wes Wellness) regarding how medical/health information about you may be used and disclosed, how you can get access to this information, and how your privacy is being protected.

I, Evan Boydstun of Evan Wes Wellness do not communicate directly with any third parties regarding your medical/health information. If you require records for insurance providers, workman's comp, etc., I will provide YOU with a copy of your records upon your written request. I will not send health records directly to any third party.

Please Note: All lab reports, clinical session notes, and Results & recommendations presentations are uploaded into your HIPAA compliant client portal. You are free to download any and all records for the purposes of sharing with a third party.

If you would like for me to collaborate with another health care provider for the purposes of providing you with integrative care, please provide me with the necessary medical release form that permits me to converse with a specifically designated health care provider on your behalf.

Safeguards in place at Evan Wes Wellness include:

  • Limited access to facilities where information is stored.

  • Policies and procedures for handling information.

  • Requirements for third parties to contractually comply with privacy laws.

  • All medical files and records are kept on a permanent file.
